Welcome! We are excited you are booking with us to begin your journey toward healing. Our office staff will reach out to you prior to your first visit to verify insurance and payment information. Your insurance may require a preauthorization prior to paying for physical therapy visits. If a preauthorization is not completed before the initial visit, insurance may not pay for the visit, and you will be responsible for the cash pay rate. If you have Tricare, Humana, or Blue Cross Anthem, please let us know so that we can help you navigate the preauthorization process.
